Haha, amazing. I’ve never experienced a mess up like this. I’ve seen/heard a couple though, Heaven and some MTV show hosted by Ray Cokes I think, are two that come immediately to mind. I think the MTV show played the mistake during the credits if I remember correctly. Currently in a hotel room with crap WiFi so can’t check my stored files.

I'm back home and found the video. Yes, you're correct, the show was called Music Planet 2Nite hosted by Ray Cokes and was a Release era concert special in a venue called Grünspan for the German TV channel Arte, which they played April 11th 2002. The mistake is played alongside the end credits.
